Expanding on the insights of the general session plenary, this panel uplifts innovative solutions for building an equitable health care workforce within communities that will span the health care delivery system. In this breakout session, attendees will hear from local health care ecosystems and innovative thinkers on different approaches to workforce issues that will center the communities served by the delivery system.

Advancing Health Care for the Justice-Involved through CalAIM
Starting in April 2024, the health care delivery system has a first-in-the-nation opportunity to proactively link justice-involved Californians to necessary health care coverage and services before they are released from incarceration. In this panel, local health care leaders pioneering the planning and implementation of the groundbreaking Justice-Initiative under CalAIM will share their insights, strategies, and visions for creating a more just health care delivery system for justice-involved communities across the state.

Advancing Equitable Care to Support Healthy Aging for California’s Older Adults
CalAIM is a transformative initiative designed to improve the quality of care and equitable access for populations of focus, including older adults. This panel will offer insights from advocates and local health care delivery system leaders on the pain points of navigating the health care delivery system for duals (Medi-Medi), how CalAIM implementation can change the landscape for older adults and their care, and what implementation improvements will be needed to meaningfully create a more equitable, accessible health care delivery system for older Californians.

Health, Equity, and Broadband for Tribal Communities and Beyond
In an era where digital resources are crucial, we invite you to participate in this dynamic and interactive workshop. Here, we will explore effective strategies and inspirational success stories for narrowing the digital gap, specifically focusing on connecting tribal communities with broadband and essential connectivity. Many in California’s Tribal communities face challenges in accessing physical health care infrastructure and a sufficient health workforce. These challenges are exacerbated by their increased vulnerability to the digital divide. During this workshop, you will have the opportunity to collaborate with community broadband experts by brainstorming policy solutions that can pave the way for telehealth and virtual care to bring much-needed health care services to tribal communities across the state.

Local Innovations: Strategies that Meaningfully Address Social Determinants of Health
Explore the impact of local innovations throughout the state in addressing the Social Determinants of Health (SDOH). Major health care transformation efforts, including CalAIM, are instrumental in integrating whole person approaches to improve the health care experience. It is crucial to tap into the creativity, expertise, and community-based solutions that contribute to better, healthier lives for California communities.
