Starting in April 2024, the health care delivery system has a first-in-the-nation opportunity to proactively link justice-involved Californians to necessary health care coverage and services before they are released from incarceration. In this panel, local health care leaders pioneering the planning and implementation of the groundbreaking Justice-Initiative under CalAIM will share their insights, strategies, and visions for creating a more just health care delivery system for justice-involved communities across the state.

CalAIM is a transformative initiative designed to improve the quality of care and equitable access for populations of focus, including older adults. In an era where digital resources are crucial, we invite you to participate in this dynamic and interactive workshop. Here, we will explore effective strategies and inspirational success stories for narrowing the digital gap, specifically focusing on connecting tribal communities with broadband and essential connectivity. Many in California’s Tribal communities face challenges in accessing physical health care infrastructure and a sufficient health workforce. These challenges are exacerbated by their increased vulnerability to the digital divide. During this workshop, you will have the opportunity to collaborate with community broadband experts by brainstorming policy solutions that can pave the way for telehealth and virtual care to bring much-needed health care services to tribal communities across the state.
